What Is All-on-4?

All-on-4 is a full-arch dental restoration technique that replaces all upper or lower teeth using just four dental implants to support a fixed prosthesis (permanent denture).

Key Features

  • Four implants per arch: Two anterior (front) implants placed vertically, two posterior (back) implants angled up to 45°
  • Fixed prosthesis: Non-removable bridge with 12-14 teeth, looks and functions like natural teeth
  • Same-day teeth option: Temporary prosthesis often placed same day as implant surgery
  • Bone grafting often unnecessary: Angled posterior implants utilize existing bone, avoiding sinus lifts

The technique was developed by Portuguese dentist Paulo Malo in the 1990s and has extensive clinical research supporting its effectiveness.

The All-on-4 Procedure: Step by Step

Phase 1: Consultation and Planning (1-2 visits)

  1. Initial evaluation: Oral exam, medical history review, discussion of goals
  2. 3D imaging: CBCT (cone beam CT) scan to assess bone density and anatomy
  3. Treatment planning: Digital planning of implant positions, angulations, and prosthesis design
  4. Medical clearance: May be required if you have significant health conditions

Phase 2: Surgery Day

  1. Anesthesia: Local anesthesia, IV sedation, or general anesthesia (patient choice + provider recommendation)
  2. Extractions (if needed): Removal of any remaining teeth in the arch
  3. Implant placement: Four implants surgically placed according to pre-planned positions (2-3 hours typically)
  4. Abutment attachment: Connector pieces attached to implants
  5. Temporary prosthesis: Same-day temporary teeth (acrylic) attached to implants

Note: You leave with functional teeth the same day in most cases. "Teeth in a Day" is a common marketing term for this protocol.

Phase 3: Healing Period (3-6 months)

Phase 4: Final Restoration

  1. Impressions and bite records: Precise measurements taken once implants fully integrated
  2. Final prosthesis fabrication: Custom-made bridge (zirconia, acrylic, or hybrid materials)
  3. Delivery appointment: Temporary prosthesis removed, final restoration attached
  4. Bite adjustment: Fine-tuning for comfort and proper occlusion
